Terms of Trade
By requesting an appointment or attendance to a small group class with Restore Prehabilitation & Recovery, you acknowledge and accept the following;
- General Terms & Conditions
- Privacy Policy – Cancellation Policy (below)
- Uncompleted Intake Forms and Waiver Policy (below)
These are automatically sent out to you via our Patient Management System upon booking and within appointment reminder emails.
Cancellation Policy
- All appointments (excluding Physiotherapy & small group classes) can be cancelled upto 48 hours proir to the scheduled appointment date with no applicable charge.
- Any appointments (excluding Physiotherapy & small group classes) cancelled within 48 hours of the scheduled appointment date may incur the full session costs and will be classified as a late cancellation.
- Attendance of any appointment more than 10minutes post the scheduled appointment time will be classified as a late cancellation and may incur the full session cost, unless informed prior via phone, SMS or email.
- Physiotherapy & Small Group Class Cancellations can be cancelled upto 24hours prior to the start of the scheduled class
- Any cancellation within 24 hours may incur the full Physio or class cost and will be classified as a late cancellation.
- Any late cancelled appointment or small group class will have an invoice raised and emailed on the same day.
- All invoices are payable upon receipt.
- Any invoice outstanding 7 days of creation are liable to incur 18% compound interest per day it remains outstanding.
- Any invoice outstanding more than 7 days of creation will be passed on to third parties for recovery and all associated costs being assigned to the debtor.
Uncompleted Intake Forms and associated waivers (none physio)
- By requesting an initial appointment with Restore Prehabiliation & Recovery you accept and will complete all requested intake forms, client waivers and read the client privacy policy, 48 hours before the scheduled appointment date.
- By not completing the requested forms 48hours prior to the scheduled appointment date, you understand the appointment can not proceed and will be classed as a late cancellation.
- All late cancelled initial consultation appointments due to incomplete intake forms will align with the above cancellation policy.
Physiotherapy uncompleted Intake Forms and associated waivers
- By requesting an initial physio appointment with the nominated Restore Prehabiliation & Recovery Physiotherapist you accept and will complete all requested intake forms, client waivers and read the client privacy policy, ASAP before the scheduled appointment date and/ or time.
- By not completing the requested forms prior to the scheduled appointment date & time, you understand the appointment may not be able to proceed and will be classed as a late cancellation.
- All late cancelled initial consultation appointments due to incomplete intake forms will align with the above cancellation policy.
Physiotherapy ACC Claims
- For all ACC claims, you accept that in the instances where by ACC decline your treatment claim, for whatever reason, you will remain liable for all associated appointment costs.
- An invoice will be raised for the appointment accordingly and emailed out.
- All invoices are payable upon receipt.
- Any invoice outstanding 7 days of creation are liable to incur 18% compound interest per day it remains outstanding.
- Any invoice outstanding more than 7 days of creation will be passed on to third parties for recovery and all associated costs being assigned to the debtor.
Gift Vouchers
- All gift vouchers are not redeemable for cash.
- There will be no replacements provided if gift vouchers are lost, stolen, destroyed or used without permission.
- Gift Vouchers cannot be combined with other vouchers or forms of payments and is not valid without promos or events.
- Vouchers are generally valid for 12months, unless otherwise stated.
- Vouchers must be reedemed before the expiry date without exception.
- We advise you to book well in advance to ensure the date of your choice and the voucher can be redeemed prior to its expiry date.
- It is the recieptant of the voucher, whos remains responsible to ensure it is redeemded prior to expiry.
Osteopathy
- Osteopathy services are delivered by Osteopathy Works Ltd. They are not legally associated with Restore Prehabilitation & Wellness Ltd and operate as a separate entity.Restore Prehabilitation & Wellness Ltd cannot be held responsible for any service delivered by Osteopathy.
- Works, including cancelled appointments, associated charges or issues following treatment.
